Housing Navigation Specialist

As part of the Cal-AIM team, the Housing Navigation Specialist assists clients in breaking the cycle of homelessness by providing clients with a full range of housing navigation support: providing support to people experiencing housing insecurity and connecting them to a comprehensive continuum of homelessness prevention, community outreach, placement assistance, individualized case management supportive services, interim housing, and permanent supportive housing. WFFRC is seeking candidates with case management experience to join our program as a Housing Navigation Specialist.

Job Description


Community Health Worker/ Navigator II

West Fresno Family Resource Center (WFFRC) is seeking a Community Health Worker (CHW)/Navigator to implement services for its Health Disparities program, targeting underserved communities, especially the Hmong community. The Health Disparities program provides community health support and promote health intervention activities for implementation of Fresno County’s Initiative to address COVID-19 related health disparities. The CHW will conduct care coordination services including assessments to identify clients need with the goal of linking them to services that will improve their health and wellbeing. This program is funded by Fresno County Department of Public Health.

Community Health Administrative Assistant II

The Community Health Administrative Assistant II will perform general administrative support for West Fresno Family Resource Center by conducting and organizing administrative duties and activities including receiving and handling information, providing front office assistance and staff support for community health programs.


The candidate will perform other general clerical and administrative duties as requested, which may include maintaining database, recordkeeping, assisting with weekly and monthly reports, maintaining office supplies and coordinating equipment maintenance. The ideal candidate is highly motivated, organized, and outgoing.
